Welcome & Purpose
The Resident Success Plan is designed to help each resident establish personal goals and develop a clear path toward stability, independence, and successful reentry into the community.
Gholston Holding LLC will work with residents to identify goals related to housing, employment, education, treatment and recovery, financial stability, health, family relationships, and other areas important to successful independent living.
Each resident is encouraged to actively participate in developing and following their individual success plan.
Personal Goals & Action Plan
Each resident will be encouraged to identify personal goals and the steps needed to achieve them. Goals may be reviewed and updated as the resident progresses through the program.
Goals may include:
• Obtain or maintain employment
• Complete education, GED, or job training
• Participate in required treatment or recovery services
• Develop a monthly budget and improve financial stability
• Obtain identification and important personal documents
• Maintain compliance with parole, probation, or supervision requirements
• Improve physical and mental well-being
• Strengthen positive family and community relationships
• Develop independent living and life skills
• Locate safe and stable permanent housing
• Establish a plan for successful transition from the program
Progress Reviews
Residents will periodically review their progress toward their individual goals. These reviews may be completed with Gholston Holding LLC staff and, when appropriate, with parole or probation officers, treatment providers, case managers, or other approved service providers.
Progress reviews may include:
• Employment or job-training progress
• Education or GED progress
• Treatment and recovery participation
• Compliance with parole, probation, or supervision requirements
• Financial stability and budgeting
• Housing goals
• Attendance at required appointments
• Development of independent living skills
• Barriers or challenges that may require additional support
• New goals or changes to the resident’s existing success plan
The Resident Success Plan may be updated as needed to reflect the resident’s progress, changing needs, and long-term goals.
Resident Commitment
Each resident is expected to take an active role in working toward the goals identified in their Resident Success Plan.
Residents are encouraged to:
• Attend scheduled meetings and appointments
• Communicate regularly with staff and service providers
• Follow through with referrals and recommended services
• Comply with parole, probation, treatment, or supervision requirements
• Inform staff of challenges or barriers affecting their progress
• Work consistently toward employment, education, financial, housing, and personal goals
• Participate in planning for successful independent living
