Move-Out & Transition Plan

Gholston Holding LLC is committed to helping residents prepare for a successful transition from the program into stable, independent, or supportive housing.

The move-out and transition process is designed to help residents plan ahead, identify appropriate housing and community resources, and continue working toward employment, recovery, supervision, treatment, and other personal goals.

Residents are encouraged to begin planning for their transition before their expected move-out date.

The goal is to help each resident leave the program with a safe and realistic plan for continued stability and success.

Transition Planning

Residents are encouraged to work with Gholston Holding LLC staff, supervising agencies, treatment providers, case managers, and other community partners to develop an appropriate transition plan.

Transition planning may include:

  • Identifying permanent or supportive housing
  • Maintaining employment or seeking employment
  • Continuing substance-use recovery or treatment services
  • Continuing mental health or counseling services when applicable
  • Following parole, probation, or other supervision requirements
  • Arranging transportation
  • Establishing a personal budget and financial plan
  • Obtaining identification and important personal documents
  • Connecting with healthcare and community resources
  • Developing a support network for continued stability

Residents are encouraged to actively participate in their transition planning and communicate any barriers that could affect a successful move-out.

Move-Out Preparation

Before leaving the program, residents should make reasonable efforts to complete the necessary steps for a safe and organized move-out.

Residents should:

  • Confirm their expected move-out date with staff
  • Provide their forwarding address or contact information when available
  • Remove all personal belongings from the property
  • Return all keys and other Gholston Holding LLC property
  • Leave their bedroom and shared areas clean and free of personal property
  • Take medications, identification, important documents, and personal records with them
  • Confirm transportation to their next housing location
  • Notify parole, probation, treatment providers, or other supervising agencies of their new residence when required
  • Complete any applicable move-out paperwork with staff

Residents should speak with staff as early as possible if they need assistance preparing for their move-out.

Housing & Community Connections

Before move-out, residents are encouraged to identify housing and community resources that can help support continued stability.

Depending on the resident’s needs, resources may include:

  • Permanent or supportive housing programs
  • Employment and job-placement services
  • Education or vocational training
  • Substance-use treatment and recovery support
  • Mental health and counseling services
  • Medical and healthcare services
  • Food assistance and other public benefits
  • Transportation assistance
  • Parole, probation, or reentry services
  • Community and peer-support programs

Gholston Holding LLC may provide residents with information about available community resources and referrals when appropriate.

Residents are responsible for following through with referrals, appointments, applications, and other requirements necessary to access services.

Personal Property & Final Room Check

Before leaving the program, residents are responsible for removing their personal belongings and completing a final check of their living area.

Before move-out, residents should:

  • Remove all clothing, personal items, food, and other belongings
  • Check closets, drawers, cabinets, bathrooms, and storage areas
  • Remove trash and dispose of it properly
  • Leave the bedroom and shared areas reasonably clean
  • Return keys and any other property belonging to Gholston Holding LLC
  • Report any property damage or maintenance concerns to staff
  • Participate in a final room or property inspection when requested

Residents should make arrangements with staff in advance if they have questions regarding personal property or the move-out process.

Personal belongings left behind after move-out will be handled in accordance with applicable law and Gholston Holding LLC policies.

Program Completion & Departure

Residents leaving the program are encouraged to complete the move-out process in cooperation with Gholston Holding LLC staff.

Before departure, staff and the resident may review:

  • The resident’s transition plan
  • Housing arrangements after leaving the program
  • Employment, education, treatment, or recovery plans
  • Parole, probation, or other supervision requirements
  • Community resources and referrals
  • Outstanding program responsibilities
  • Return of keys and Gholston Holding LLC property
  • Forwarding contact information when available

Gholston Holding LLC may document the resident’s departure date and completion of the move-out process for program records.

Successful transition planning helps residents continue building stability, independence, and long-term success after leaving the program.
